Title: Model Endorsements and Declarations Relating to Article 107 of the Insurance Act (2004 . 06 . 30 Amended)
 
 Article 1 Article 1
These endorsements apply to any contract of this company for life insurance or personal injury insurance that takes effect on or after 11 July 2001, and to any contract of this company for simple life insurance proposed on or after 1 October 2003.
These endorsements constitute part of this contract (or rider). Any part of the contract (or rider) that conflicts with these endorsements shall be invalid.

Article 2
If, at the time this contract (or rider) is entered into, the insured is a minor who has not yet reached his or her fourteenth birthday, or is mentally impaired, or is of diminished mental capacity, any insured amount for death (regardless of the method of settlement or what they are called) becomes an insured amount for funeral expenses.

Article 3
Where the insured is a minor under fourteen years of age, or mentally impaired, or of diminished mental capacity, the aggregate insured amount (not limited to that of this company) under funeral expense insurance applied for on or after 11 July 2001 may not exceed the maximum amount for funeral expenses as set by the competent authority. This company is not liable for settlement of any amount in excess thereof, and shall refund without interest any insurance premiums already paid for such excess amount.

Article 4
Under the circumstances contemplated in the preceding article, if the proposer applies for insurance with two or more companies and the aggregate insured amount for funeral expenses as applied for exceeds the limit set forth under the preceding article, this company shall, within the scope of the funeral expense amount that it has underwritten, settle the insured amount for funeral expenses in accordance with policy stipulations, with the settlement commencing from the earliest proposal and continuing, in the chronological sequence of the proposal time specified in each proposal, until the maximum amount for funeral expenses as set by the competent authority has been paid out.
The "maximum amount for funeral expenses as set by the competent authority" referred to in Article 3 and the preceding paragraph is as follows:
1. On or before 31 December 2002 -- NT$1 million.
2. On or after 1 January 2003 -- adjusted to NT$2 million.
3. The insured amount for funeral expenses stipulated in a simple life insurance contract proposed on or after 1 October 2003 shall be counted into the combined total.
Under the circumstances contemplated in paragraph 1, if the proposal time for insurance contracts with two or more companies is identical or it is not possible to determine their chronological sequence, each such company shall bear its respective liability on the basis of the proportion between the amount of funeral expenses that it has underwritten and the difference remaining after the funeral expense amounts payable by any companies with an earlier proposal time are subtracted from the maximum amount for funeral expenses.
Declarations and Notifications:
□1. I declare that I am aware of and understand the content and stipulations of these endorsements.
□2. With respect to life insurance contracts (or riders), personal injury insurance contracts (or riders), and simple life insurance contracts proposed on or after 1 October 2003 under which the insured is the same person as the insured under this contract (or rider), the aggregate (as calculated per the stipulations of this contract) insured amount (not limited to that of your company) for funeral expenses is NT$_______.(Note) This figure does not exceed the maximum amount for funeral expenses as set by the competent authority (NT$2 million as of 1 January 2003).
□3. I have carefully read the pertinent provisions of the Insurance Act attached hereto.
□4. With respect to the insured amount for funeral expenses in any life insurance, personal injury insurance, and simple life insurance contracts that I have applied for on behalf of the insured under this contract, in addition to policies applied for with your company, such insured amounts are also provided for in the following insurance contracts:

Note:
The insured amount for funeral expenses as provided for under this contract (or rider) will not be aggregated with insured amounts for funeral expenses and/or death as provided for under the following insurance contracts:
1. A student group insurance contract arranged by the Ministry of Education of the Executive Yuan, the Taipei City Government, or the Kaohsiung City Government.
2. A resident group insurance contract arranged by the government of a rural township (siang), urban township (jhen), or city.
3. A life insurance or personal injury insurance contract that took effect before 11 July 2001, and any simple life insurance contract that took effect before 1 October 2003.
4. A life insurance or personal injury insurance contract (or rider) for one-year term insurance with a clause of guaranteed renewal, applied for before 11 July 2001, with the renewal date falling after 11 July 2001.
5. A health insurance contract (or rider). (If the benefits under a health insurance contract (or rider) include death benefits provided for accidental death, such benefits shall be aggregated with the amount to which the funeral expense limitation applies.)
6. An insurance contract not governed by the Insurance Act or the Simple Life Insurance Act.
